海外渡航に必要な書類

各書類英語での記入作成は5,000円（税抜）となり、記入に数日かかりますのでお早めにご準備ください。

・狂犬病ワクチン/混合ワクチン接種証明書
動物検疫所にて申請
・輸出検査申請書：検査希望日の10日前までに、NACCSまたはメール（提出先はこちら：犬や猫などペットの輸出入問合せ：動物検疫所 ）により申請してください。
・輸出検疫証明書：EU諸国や豪州などでは事前署名が必要になるので検疫所にお問い合わせください。

Pet Travel Certificates for International Travels
Travelling with your pet, especially abroad, is a complex and time consuming process.
Our experienced and English speaking staff members at HALU Animal Hospital, will help you through this process, providing you with the documentation, as well as tests and medication necessary for travel.
Keep in mind these procedures may be complex and time consuming depending on the destination, so please plan in advance.

Documents Required for Overseas Pet Travel
Our clinic can prepare each of the following documents in English.
The fee for preparing each document in English is ¥5,000 (excluding tax). Preparation takes several days, so please arrange these well in advance.

・Microchip implantation certificate
・Rabies vaccination / combination vaccination certificate
Apply through the Animal Quarantine Service:
・Export inspection application: Please apply via NACCS or email no later than 10 days before your desired inspection date (submit to: Animal Quarantine Service – Inquiries about pet (dog/cat) export and import).
・Export quarantine certificate: Some countries, such as EU member states and Australia, require advance signature, so please check with the quarantine office.
